If you can not referee a game, you are responsible to find your own
replacement.  If you send a ref an email asking them to cover for you- make
sure they confirm that it is OK!  Thank you!

Update to Pay Procedure
Referees will need to fill out a Stateline Game Report 2009 (*NEW*, rules for form are
below; forms will also be in the Concession Stand) and leave the report in the manila
envelope in the Concession Stand after the game.  Assistant Referees need to make
sure their names are on the Report and it is turned in.  The following week the ESC
Treasurer will use your Game Report  to write you a check for the games you referred.  
Your check will be left in the Concession Stand for you to pick up.  IF YOU CAN NOT
REFEREE A GAME, PLEASE EMAIL OR CALL HOYT ASAP!  
Rules for Use of Stateline League Game Report
Purpose for revised game report:
The Stateline Board of Directors, comprised of district officers and representatives of all clubs participating in the league, has
considered the best way to address the issue of coach conduct and responsibilities for rosters, player passes, and coaching
licenses as well as referee responsibilities for completing game reports and checking for coaching licenses, player passes, and
proper player equipment/uniform. In order to collect information and maintain records about the games and teams in the Stateline
League, the Board decided to create a new League Game Report Form to be completed for each game. This form will be
completed by the coaches and the referee and then sent to a central location for processing.
Responsibilities of the Home Coach:
The Home Coach is required to bring the following items to each game:
• Official roster (provided by the club’s registrar) listing every player that will play with the team that day. This roster must list each
player with their jersey number.
• A copy of the Stateline League Game Report.
• All player passes for those listed on the roster that will be playing that day.
• Coach’s license (each coach present at the team benches must present a valid coach’s license and be listed on the team
roster).
Home Coach responsibilities on the game report:
At the game, the home coach will fill out the required blanks on the game report, transfer the game report to the visiting coach to fill
in the pertinent information, and make sure the game report gets to the referees along with the rosters for each team.
The following blanks must be filled out completely before turning the game report over to the referees: Date of game, Time of
Game, Age, Division, Site, Field, Length of Game, Game #, and in the Home Team box: Team name, Jersey color, Coach’s name
and telephone, Manager’s name and telephone.
Responsibilities of the Visiting Coach:
The Visiting Coach is required to bring the following items to each game:
• Official roster listing every player that will play with the team that day. This roster must list each player with their jersey number.
• All player passes for those listed on the roster that will be playing that day.
• Coach’s license (each coach present at the team benches must present a valid coach’s license and be listed on the team
roster).
Visiting Coach responsibilities on the game report:
At the game, the visiting coach will fill out the required blanks on the game report once it is transferred from the home coach and
make sure the game report gets to the referees along with the rosters for each team.
The following blanks must be filled out completely before turning the game report over to the referees: In the Visiting Team box:
Team name, Jersey color, Coach’s name and telephone, Manager’s name and telephone.
Responsibilities of the Referee(s):
After receiving the game report and rosters from each team the referees are required to perform the following inspections:
• Equipment & Uniform check of players on both teams.
• Check player passes against team rosters for both teams.
• Check each coach’s license: checking for a state issued coaching pass and noting the level of each coach’s license.
At the completion of the game the referees are required to:
• Return all player passes to each coach, pulling any passes of players who were ejected from the game (received a red card) if
such a case occurred. Player passes pulled due to ejections must be sent with the game report and the referees shall notify the
referee assignor who gave them the game of the situation.
• Fill in final game scores and misconduct (Yellow/Red cards) for each team.
• Complete ALL fields required as stated on the game report under “TO BE COMPLETED BY REFEREE.” All referees present for
the game must be listed. ALL information is necessary to maintain accurate records on teams, coaches, and referees for league
purposes.

Some clubs and referee assignors may have the referees send game reports to a different location. If the referee has been told to
do this, it will then be the responsibility of the club or referee assignor to send each game report to the location stated on the game
report with in 48 hours.**

**Elkhorn Soccer Club requires that the head referee for each game turn the completed game report into the Rotary Concession
Stand at the end of the day.
